I am going to brew a 5BBL batch. When I have beersmith tell me the steps it says:

"add 13.28 BBL of water at 154f"

I have tried changing various equipment profile setting. I have 275 lbs of grain in the grain bill.

That's 411.68 gallons of water at strike. the total water listed on this recipe is 187.23.

What might I have set wrong?

We would need to see your equipment profile and the water balance tab of your recipe to make any meaningful recommendations. If you have made an equipment profile which matches your process parameters, the recommendations should be driven by those settings. If there is something else in the process which drives the water demand higher, that might also explain the volume of the recommendation.

Water balance? I assume that is the Vols tab. Ok. That helps. Never saw that tab.

Now we're talking 183 gallons, much better than 13 BBLs
