bottle conditioning/carbonation

One of my 22-oz bottles in a case was extremely wild and overcarbonated, while all the others were normal. What might cause this?

Sounds like you had a infection in that bottle, I would wager something was not sanitized properly, if no off flavors exist in the other bottles then its likely that one picked up a infection.
 
Thank you. That must be the cause. Although bottles I was using looked very clean, and I used StarSan, I did not brush out first with cleaner, which I will surely do next time.
